2025 Form 1095-C Now Available in Employee Self-Service

Dear Chapman Community,

The 2025 Form 1095-C is now available electronically in Employee Self-Service as of Friday, Feb. 27, 2026.

What is Form 1095-C?
Form 1095-C allows the Internal Revenue Service (IRS) to confirm Chapman’s compliance with the coverage and affordability provisions of the Affordable Care Act (ACA).

All Chapman employees who were eligible to participate in medical insurance benefits in 2025 should receive Form 1095-C — including those who declined coverage.

Background
The ACA requires many employers to provide Form 1095 to benefits-eligible employees and to report to the IRS that medical coverage meets affordability standards. The form is issued annually for the prior tax year and includes information about your medical coverage, who was covered and when.

Because there are three versions of Form 1095, you may receive more than one. Below is a brief overview to help clarify the differences:

Form 1095-A
Issued by the Health Insurance Marketplace (for example, Covered California for California residents) to individuals who enrolled in marketplace coverage during the 2025 calendar year.

Form 1095-B
Issued by your medical insurance carrier. Employees enrolled in Kaiser Permanente or Cigna plans in 2025 will receive this form directly from the carrier.

Form 1095-C
Issued by Chapman University. This form is provided to employees who were considered full-time benefits eligible under ACA standards (those working 75% time or more) during the 2025 calendar year.

Example:
If you were a full-time Chapman employee enrolled in Kaiser in 2025, you should receive:
• Form 1095-B from Kaiser
• Form 1095-C from Chapman

How to Access Your Form
You may download your 2025 Form 1095-C by navigating to:

If you did not consent to electronic delivery, a physical copy will be mailed to the address on file in Employee Self-Service.
